Bachelor Level Social Worker / Case Manager - Montcalm Care Network
MCN is seeking a Case Manager. Work in an organization that supports professional growth, values team-based interventions and provides trauma-informed care. Montcalm Care Network serves adults and children with severe mental illness and intellectual developmental disabilities while specializing in models of integrated health and co-occurring treatment. MCN is the respected leader of behavioral health care in our community and actively partners other community providers. Competitive pay and benefits with average pay for this position ranges from $51,894 to $73,980 depending on experience.
Case Managers provide community-based outreach to persons with mental illness and are responsible for linking, coordinating, monitoring and psychoeducation. Our case management model utilizes a team approach with nurses, peer supports, and paraprofessional staff providing comprehensive care. Position will provide services to populations of adults with severe mental illness or intellectual developmental disability.
This position may provide case management services for individuals with intellectual development disability (IDD) and severe mental illness (SMI) primarily residing in an Adult Foster Care home. Primary responsibilities include supporting and providing comprehensive oversight of care for individuals residing in contracted AFC homes and facilitating transitions toward community-based living.

Employment Qualifications:
Education: At minimum possession of a bachelor’s degree in social work or a related human services field
Experience: One year of professional experience working with persons with intellectual/developmental disabilities or severe mental illness
Other Requirements: Licensure or limited licensure as a Social Worker or must be a qualified CMHP and/or QIDP or supervised by individual with this credential.
Possession of a valid State of Michigan driver's license and ability to obtain and maintain a chauffeur’s license within 30 days of hire.
Local travel is an essential function of this role.

Montcalm Care Network is looking to hire a Family Support Assistant. Join an innovative workforce dedicated to the provision of evidence based, integrated mental health care. MCN serves persons with mental illness and developmental disabilities with an emphasis on trauma informed care and co-occurring substance use treatment.
The Family Support Assistant (FSA) provides support services to children with serious emotional disturbances and/or developmental disabilities and their families who are receiving an intensive community-based service. Assists children in the development of skills to promote age-appropriate independence and pro-social behaviors, as well as promoting community inclusion. Attends to the health and safety needs of the family. Assists parents in the development of consistent strategies and routines to address needs and behaviors identified in the family centered plan. Promotes mental health recovery, wellness, and full participation within the community for consumers in a manner where practice is trauma informed, co-occurring capable, person/family centered and integrated with other health care providers.
This is an hourly position where evenings are required, some weekends

Montcalm Care Network (MCN) serves adults and children with mental illness and developmental disabilities while specializing in models of integrated health and co-occurring treatment.
Master level
Candidates must be advanced standing or second year internship. Opportunities available in Community Mental Health setting in multiple departments. Population choices include adults or children with mental illness or developmental disabilities.
Department choices include:
- Access – Learning advanced assessment skill set, crisis intervention, and system navigation.
- Outpatient – Psychotherapy using cognitive behavioral interventions with exposure to evidence-based practices such as DBT and EMDR.
- Children’s Home-based or Infant Mental Health – Intensive intervention with children and/or infants, providing family psychotherapy, case management and coordination with various systems.
- Intensive Case Management – Community based interventions with adults having severe and persistent mental illness and co-occurring substance use disorders.
Bachelor level
MCN will consider bachelor level students in the social work program. Placements would be considered in case management or prevention areas. Must be in final internship placement. Opportunities in a Community Mental Health setting available in Case Management with either the adult population with mental illness, or child/adult population with developmental disabilities. Community based interventions to link persons to necessary supports, coordination with multiple systems and monitoring mental health stability.
